Position Title:              Social Worker

 

Responsible to:           Principal or Director of Student Services

 

General Duties:           Deliver individual, group and family counseling and casework services to identified day treatment youth

 

Performance Responsibilities:

 

  1. Maintain a log of activities
  2. Collect data and maintain a file on each student per DJJ mandates
  3. Demonstrate high expectations for every student.
  4. Collect and submit requested data, program assessment/evaluations per DJJ guidelines
  5. Arrange, conduct, and facilitate monthly parent/family meetings.
  6. Collaborate with the Family Resource and Youth Services Center Director
  7. Identify community partners and establish working relationships.
  8. Attend professional development trainings
  9. Provide or refer student to services and resources to meet all student needs.
  10. Participate in district sponsored meetings as assigned.
  11. Assess the students’ physical, social/emotional, and economic needs.
  12. Provide family assessments as needed.
  13. Develop an individual treatment plan ITP to address assessed problems.
  14. Schedule and plan any treatment planning meetings with students and parents
  15. Monitor targeted students’ attendance, and intervene when necessary to assure regular attendance.
  16. Establish a desirable working relationship with parent/guardian to assure constructive parental participation and involvement.
  17. Participate in plan of school wide supervision of students
  18. Develop a comprehensive knowledge of all community agencies and their resources.
  19. Attend and participate in individual supervisory, counselor, and teacher conferences upon request.
  20. Confer with the principal upon request.
  21. Respond to students’ crises by working with school staff and outside agencies to administer interventions for:
    • Suicide risk
    • Physical/sexual abuse, neglect
    • Deaths of student, family member, faculty
    • Conflict resolution

Evaluation:
